Model Release Form

A model release form is a contractual document that specifies how you can use someone’s image and likeness in your marketing. This document may be very specific or very general. Have models, students, teachers, performers and others sign this before using their image in your marketing.

This is a sample below, work with your appropriate legal counsel to update as necessary for your business needs.

hereby grant <FULL LEGAL NAME OF BUSINESS>, and assigned photographers and videographers as well as his/her legal representatives (including but not limited to any agency, client or publication), irremovable permission to publish photographs and videos of me taken during:  <WHEN/WHERE THE PHOTOS/VIDEOS WERE TAKEN>

These photographs/videos may be published in any manner, including (but not limited to) advertisements, Facebook/other social media/portfolio site, other online/digital media, print or video publications or media. Furthermore, I will hold harmless the aforementioned photographers and videographers and his/her legal representatives and assigns, from any liability by virtue of photo retouching, cropping, collaging other editing or other combinations with other media/text/video/etc. that may occur in reproducing/using this photograph and/or video.

I agree that I have no rights to the content, and all rights to the content belong to the photographers and videographers and his/her assigns. I acknowledge and agree that I have no further right to additional consideration or accounting, and that I will make no further claim for any reason to photographers and videographers and his/her assigns. I acknowledge and agree that this release is binding upon my heirs and assigns. I agree that this release is irrevocable, worldwide and perpetual, and will be governed by the laws (excluding the law of conflicts) of <WHERE YOUR BUSINESS IS REGISTERED>

I affirm that I am 18 years of age or older, and competent to sign this release on my own behalf. I have read this release and fully understand its implications. If younger than 18 years of age, a parent or guardian has read this release and fully understands its implications and has signed on my behalf.
